For Individuals BORN in Jefferson County, Indiana

This NEW Online Service will allow applicants to complete an online order and also track their orders in real-time.   This service will also allow for multiple shipping and pickup options.

Multiple Shipping options include: USPS; UPS; and FedEx.   If you have selected to have your order shipped to you – payment must be in the form of Debit/Credit Cards.  Convenience & Credit Card fees do apply.

In-Office Pickup Option: If you have selected to pick up your order at the Health Department Office – you will be notified when your order is ready for PICKUP.  During the completion of your online order and you chose the “PAY AT COUNTER” option – this will allow you to make your payment here in the office when your order is ready for pickup (Cash, Check, or Money Orders ONLY). 

If you wish to pay with a Debit/Credit Card you would need to choose the “CHECKOUT” option and provide your credit card information at the time of placing your online order (Convenience & Credit Card fees do apply). Once your debit/credit card payment has been processed, you will be notified when your order is ready for PICKUP.

Note: Other documents may be accepted at the discretion of the Registrar. A person who has no form of Identification, should ask someone on the eligible persons list to obtain the record.

Eligibility

Here is a list of persons eligible to obtain certified copies of birth certificates.

Legitimate Birth Records

  • Attorney representing person named on record (must have I.D. for self and I.D./permission note from person named on record)
  • Brothers and Sisters
  • Caseworker from Division of Family and Children with court appointed guardianship papers
  • Children
  • Court Appointed Legal Guardian (must provide guardianship papers)
  • Grandparents
  • Individual named on the record (must be over age 18)
  • Law Enforcement personnel with I.D. and court order
  • Other county agencies left up to discretion of the registrar and the local health department
  • Parents of the person named on the record
  • Spouse

Illegitimate Birth Records

  • Individual named on the record (must be over age 18)
  • Mother of the person named on the record
  • Father of the person named on the record (name must appear on the record)

Written Statement

Note: If an individual under the age of 18 wishes to request a certificate, he / she must have a written permission note from a parent or guardian, authorizing the release of a birth certificate to a specified person.The written statement must include a parent or guardian's signature and be accompanied by acceptable I.D. The individual under age 18 must also provide acceptable I.D.
